ATSB report finds Qantas A380 flew two sectors with work light inside wing
The Australian Transport Safety Bureau (ATSB) has found that a maintenance work light was left inside the left wing of a Qantas Airbus A380 during scheduled work at Sydney Airport (SYD), and that the aircraft went on to fly two international sectors before the tool was recovered.
The final report into investigation AO-2026-004, released on August 17, 2026, states the light was inadvertently left in the wing following maintenance on the aircraft’s air conditioning system on January 7, 2026. The A380 was subsequently flown to Dallas/Fort Worth International Airport (DFW) and back to Sydney, and the light was removed on January 9, 2026.
The aircraft, an A380-842 registered VH-OQK, sustained no damage, and no injuries were reported.
Three checks fail to catch missing work light

Three separate opportunities to catch the missing light passed without it being flagged. Maintainers “did not find the light during their foreign object clearance inspections” at the end of the work, ATSB Chief Commissioner Angus Mitchell said, and the light’s absence was not noticed when the maintainer returned tools, or during the unreturned tool check at the end of the shift.
A licensed aircraft maintenance engineer then issued a certificate of release to service while the work light remained unaccounted for.
Second Qantas tool control finding in two years
The finding follows an earlier ATSB report, published in November 2024, into a compressor turning tool left in an engine of another Qantas A380, registered VH-OQI, at Los Angeles International Airport (LAX) in December 2023.
